Kelsey-Seybold Clinic is Houston’s premier multispecialty group practice, founded in 1949 by Dr. Mavis Kelsey in Houston’s famous Texas Medical Center. More than 500 physicians and allied health professionals practice at 30 locations and an accredited Sleep Center in the Greater Houston area. Medical services offered by Kelsey-Seybold include medical care in 55 medical specialties including primary care and specialty care, outpatient surgery centers, an accredited Sleep Center, a Radiation Therapy Center, laboratory services, advanced radiology services and other diagnostic services, 15 on-site Kelsey pharmacies, and a secure web portal for patients to communicate with their Kelsey-Seybold doctors, get test results and schedule appointments.

This position works under the supervision of the Managing Physician for Population Health and Primary Care at Home to help provide medical care and care coordination. The APC is involved in the planning, implementation, and evaluation of medical and nursing care for patients with complex care needs. The Care Manager – Nurse Practitioner assesses the physical and psycho-social condition of patients by performing and obtaining comprehensive or problem-focused physical examinations and medical histories. The Care Manager orders collects, interprets, and evaluates laboratory and other diagnostic tests to assess patient problems and health care needs. The Care Manager may prescribe and/or recommend medical drug therapies or other forms of related therapeutic treatments and procedures in accordance with approved protocols/policy and with attention to efficacy, safety, and cost. The Care Manager formulates and implements comprehensive outcomes-based patient care plans based on patient assessments, scientific rationale, standards of care, and professional practice guidelines. The Care Manager directs other members of the team to help implement the care plan. The Care Manager initiates timely consultation and referral when the problem exceeds the scope of practice and/or expertise. This position conducts mostly home visits. About 6 home visits per day.

Qualifications include:

  • Licensed in the State of Texas.
  • Board Certified and or Board Eligible.
  • Graduate from an Accredited Masters of Science in Nursing Program.
